I'm been using a web form to collect subscriber information. If I wish to add the person's birth date, what is the format and variable name?

The variable can have any name you just need to specify it under View > Options, Web Forms (by default, it is Birthday).

The formatting needs to match your locale formatting. I.e. if you select "Region and Language" in the Control Panel, it should match the "Short Date" setting.
I can't remember whether in Canada you follow the weird US date order, if so your dates should be formatted like this (for Xmas day):
